New Patient Registration

To register at the practice you need to present with your NHS medical card with your NHS number, this can be provided by your current surgery or by the local NHS Trust. Your registration will require an attendance for a well person check and the completion of a questionnaire. 

Children under the age of 16 wishing to register must give a copy of their immunisations to the practice.

All patients registered at the practice are held by Dr Nathu who is then ultimately responsible for all care received. 

We do employ locum but they do not hold their own patient lists.
